Effective governance components and systems were not in place. Risk assessments to mitigate potential risks to patients and staff had not always been completed where required. Action had not been taken to assess and act on the opportunity risk and impact of possibly missed patient referrals to secondary care. Statutory warns had not been sent to the Care Quality Commission as required under the Care Quality Commission (plate) policies 2009.
